The Site provides general educational information about conditions, treatments, and our services. It is not medical advice and is not a substitute for professional diagnosis or treatment. Your use of the Site, including contacting us through forms or email, does not create a patient‑physician relationship.

Do not use the Site for emergencies. If you believe you have a medical emergency, call 911 or go to the nearest emergency department immediately.

Appointments, messaging, and portal access

  • Submitting a request through the Site does not guarantee an appointment; our staff will contact you to confirm.
  • Do not submit protected health information (PHI) through general forms or email. Patients should use the patient portal for clinical communications and access to records.
  • If we offer telemedicine, services may be available only to patients located in states where our clinicians are licensed.

Eligibility and user responsibilities

If you provide a phone number or email, you may receive service‑related messages (e.g., confirmations, reminders) and, if you opt in, promotional content. Message and data rates may apply. You can opt out of promotional emails at any time via the unsubscribe link and opt out of texts by replying STOP; reply HELP for help. Consent is not a condition of medical care.

Disclaimer of warranties

To the fullest extent permitted by law, the Site and its content are provided “as is” and “as available,” without warranties of any kind, express or implied. We do not warrant that the Site will be uninterrupted, secure, or error‑free, or that defects will be corrected.

Limitation of liability

To the fullest extent permitted by law, in no event will Lasha-Giorgi Esebua, its clinicians, employees, or affiliates be liable for any indirect, incidental, special, consequential, or punitive damages, or for loss of profits, data, or goodwill, arising from or related to your use of (or inability to use) the Site. Our total liability for all claims relating to the Site will not exceed one hundred U.S. dollars (US$100) or the amount you paid to use the Site, if any—whichever is greater. Some jurisdictions do not allow certain limitations, so some of the above may not apply to you.

Indemnification

You agree to defend, indemnify, and hold harmless Dr Lasha-Giorgi Esebua and its affiliates from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising from or related to your violation of these Terms or misuse of the Site.

Governing law and venue

These Terms are governed by the laws of the State of Indiana, without regard to conflict‑of‑laws principles. You agree to the exclusive jurisdiction and venue of the state and federal courts located in Vanderburgh County, Indiana, for any disputes not subject to small claims court.

Dispute resolution; small claims

Before filing a lawsuit, you agree to first contact us to attempt to resolve the dispute informally. Either party may bring claims in small claims court, if eligible.

Termination

We may suspend or terminate your access to the Site at any time, with or without notice, for any reason, including if we believe you have violated these Terms.

Changes to the Terms

We may update these Terms from time to time. The "Last updated" date reflects the latest revision. Your continued use of the Site after changes become effective constitutes acceptance of the revised Terms.

Other terms

If any provision of these Terms is found unenforceable, the remaining provisions will remain in full force. These Terms constitute the entire agreement between you and Lasha-Giorgi Esebua regarding use of the Site.
